Instructions In lieu of conducting an intake with a real client, this assignment is meant to mimic an intake session as best as possible, while still providing you with the opportunity to practice the skills of intake interviewing and data collection. Note: the hypothetical case you will develop and information you obtain in this assignment will be used to inform Assignment 2 (the role-play video session assignment in Unit 4). See the steps below for details of completing this assignment. Note: There will be no severe psychological issues for the intake session such as suicidal ideation, chemical dependency, psychosis, etc. Remember the goal of this learning opportunity is for each student to understand the process of collecting information from a client based on an intake session. a. create a client scenario with a therapist and a client for each scenario. b. Using the provided Clinical Intake Form, or one of your own choosing (many examples can be found online), complete the intake form. Typical intake forms include the following: name (hypothetical), chief complaint, symptoms, risk to self/others, substance issues, current/past psychological treatment, medications, childhood history, family history, present/past abuse, current functioning (current situation), current/past stressors, support/strengths, current/past DSM diagnoses, goals/objectives and follow-up. The provided form is just a basic template, and you are free to add or remove items as you see appropriate. Likewise, you are able to format the intake form as you wish. c. While the CTRS is not required for this assignment, you may wish to begin familiarizing yourself with it as it is required for Assignment 2 (the video session), which is informed by this assignment. Each student uploads as Assignment 1 only the intake where they assumed the role of therapist. e. While the use of the course text will be required to complete this assignment, it is not necessary to cite the text or to include a reference list. Likewise, the use of APA is not required for this one assignment, but clear, professional writing is still expected.
