Don is a Caucasian heterosexual, catholic who is of Irish and German descent. Don stated that his family being of two cultural backgrounds had some challenging influence on his life. His mother being of German and his Father being Irish for which they met when his father was in the military. However Don was born in the United States. His families were practicing Catholic for which he was raised with the strong catholic belief. This would be significant in Don present issues with both of his wives in the rearing of their kids. Don family primary language was English but his mother attempted to teach him German but hes forgotten most of it as he gotten older. Since Don didnt have to move from his family country of origin, acculturation for him wasnt an issue of concern. Since no one detect any different among his peers as it related to his parents and grandparents background.
Current Employment:
Don is a Nurse Practitioner contracting with U. S. Army
Educational Background:
Don is a nurse practitioner with a (Doctor of Nursing practice). Don also has a Master in Nursing and a bachelor of science
Military Background:
Don Served 12 Years in the U. S. Army as nurse, here is where he gathered his work experience in order to move forward in his Doctor of Nursing. He was commission as an officer, leaving the Army as Major in rank.
Presenting Problem
Developmental crises:
Don stated that he was having some headache a few times a week but was noticing the intensity and frequency was starting to increase. Don stated the he went into to see their family physician, who informed Don after a series of test; which all return back negative that he felt Don issues were more emotional and psychological stress. Don agrees and based on his doctors recommendation, he sought out our services. Don stated that he had recently be terminated from his job and that and his present wife were having major communication problems. Don expressed that these issues centered mainly on her children and his children with his previous wife.
Disabilities:
Don stated he didnt have any disabilities and had never seen anyone for mental health reasons.
4 Previous Mental Health Issues/Treatment:
Don stated that he had never seen anyone for mental health reasons.
Even though, his school counselor suggested that he see a counselor after he was in an accident during his senior year in High School. Don stated that one time he was informed he should seek counseling after a serious accident happen during his senior year for which he loss his best friend However Father didnt believe in counselor, thus Don never sought assistance.
4 Situational and environmental factors impacting the problem:
Don presented that one the major issues with him is that presently his wife is not working with him in rearing her son and daughter from a previous marriage. Her son continue to miss classes and her daughter is an adult is allow to bring her boyfriend to the home but conduct inappropriate behaviors such drinking, drugs and sexual activities in the home. This is a challenge for Don because he doesnt have much control over the situation. In addition to him being fired from his job, for sexual harassment it leaves Don in a very compromising position of having some equal say and rights in his home.
Family History
4 Who was in family of origin?
Don stated that he grew up on a farm for which his father establishes when he graduated from college. Don stated that his family wasnt a large family, which basically consisted of his Mother who was German, by which her parents raise her in Germany, which to present date they remain. Don father was Irish, but Don stated he never got to know his father parents they both died young from medical issues. Don father passed away from liver cancer and his grandmother died of lung cancer. Don father was an only child and his mother only had one brother for which they havent spoken in years due to he doesnt like Don father.
4 Describe family environments in childhood/adolescence:
Don stated that his family environment seem normal to him, his father was an authoritarian, and was very controlling but at the same time he was vey caring and supportive in any endeavor Don pursued. 5 Don Father was an environmental engineer and his mother was a Nurse. Don stated that he believe his parent got along well, for his mother was pretty traditional and spent a great deal of time when she wasnt working being a good homemaker, mother and wife. His father didnt appear to have many outside friends. Don doesnt really recall many family functions at his home.
Don stated education was a must and being wise with ones money was essential. Don felt his father was just cheap but his mother was a bit more giving and free with her money and interaction with other people. Don stated he didnt notice many arguments with his parents, for the most part he felt he had a pretty good childhood. Don stated he wasnt very popular in his adolescent years because he was small in stature but once in high school his popularity seem to change because he was a good football player. Having being a good football player, also seem to be what him and his father had in common.
4 Special circumstances or events in childhood/adolescence:
Don stated the only major event that he could recall in was his mother losing his sister during childbirth. As well as his best friend dying due to a car accident for which Don was in the car!
Personal History
4 Major life events:
Don stated that the only major events that he could recall were the loss of his sister who died at birth; which seems to devastated his parents, for he noted they were not as happy and interactive for sometime. Yet Don seems to notice they never pulled away from each other. Don believes that his parents were truly each others best friends. The other event that Don thought impacted his life was when he loss his best friend in a car accident, due to drinking and driving.
Don stated it was prom night and he and his friend and their girlfriends were hanging out, they decided to attend another party; for which they went down a dirt road. Unfortunately a deer, jump out and hit the car, his friend loss control of a wheel, and hit a tree. 6 He didn’t have his seat belt on and was expelled from the car. He died on impact. Don and the rest had minor injury. Don stated he had savior guilt and still lives with it today.
Educational Background:
Don stated that his parents both promoted educations, for which his mother was a nurse and his father an engineer. Don did well academically in school, usually maintaining a 3.5 or better through his earlier school years right into college. Previous family member such as his grandparents, aunts and uncles were not educated beyond high school.
4 Substance Abuse/Addiction Histories and Current Situation:
Don stated that he doesn’t used drugs and after the accident with his friend he rarely drinks, most on certain outings; otherwise his wives do most of the drinking. Don stated his parents didn’t drink at all, but he was told that his grandfather was a heavy drinker, along with an uncle. Don does smoke heavy, which he stated he has tried to quit many times but with the stress of his relationship and losing his jobs he always seem to go back to smoking.
Occupational and Educational History
4 Major life events:
Don stated that he seem to lose his jobs about every two to three years, and mostly because he seem to get accused of sexual harassment. Don stated that he feels that the people that accuse him of some form of harassment take his commitments out of content. Don stated when he just try to not say much of anything to females, they take him as being arrogant or non-sociable, thus he feels he just don’t know what to do, but it seem to keep happening over and over again. Which always seem to have a negative impact on his personal and financial life. Yet he seem to always financially prepared for it, so he never has loss anything or been in a financial hardship.
4 History of trauma:
Don stated that he hasnt experience any major trauma, besides losing his friend in the accident. Once he was a bit afraid after his father lost his finger during a tractor accident but his father made light it, so he did as well. He also stated that when the accident happen, his school counselor told him to seek counseling but his father wouldnt allow it, thus he never sought help for the issues.
